Where had Bashundhara City authorities failed?

The fire that devastated much of the Bashundhara City shopping complex, the biggest and brightest of its kind in the capital city should have been foreseen. The fire started on the 18th floor and only when the fire died down completely and the appropriate people went in to ascertain the damage, did we learn the extent of the destruction. Within the first few hours the fire had devoured at least five floors.
Why had the shopping complex authorities not taken adequate measures? The size of the place is immense. One can expect that accidental fires would have been taken into account. Surely there were fire fighting equipment on each floor and the right people to handle that equipment and douse the fire at the outset.
It looked as if napalm had been used. The fire was burning so fiercely and no one could check its advance. There must have been enough highly inflamable stuff in those office complexes, for instance, plastic, which is a derivative of petroleum and burns very well.
Where had the shopping complex authorities failed? This will of course be fully revealed after the official investigation, but one can say that they had failed in all respects actually.
