Medicare for Bangladeshis

Thousands of Bangladeshis of humble fortune go to Kolkata for medicare services. We often learn from our policy-makers that we are capable of giving world class medicare services within the country. If it is so, why do the Bangladeshis go to India, Singapore, Bangkok or London for treatment?
On the other hand medical professionals are reluctant to stay in rural areas where clinics, hospitals, and health centres have been built at the cost of tax payers' money. Public medical colleges where physicians had their medical schooling are also run by tax payers' money. Shouldn't we plan something so that we do not have to go abroad for healthcare services?
